1) Near Miss   a) Unplanned event that did not cause injury / damage b) material was dropped causing damage c) first aid d) employee singing while working 2) Find a co worker handling material and chains -not using no touch tool  a) coach them on the safe way to move material/chains b) do nothing they can handle it 3) Walk up to a trailer and notice a co worker standing on top of material that is not tied down or staked a) remind them of our stay grounded policy b) get your camera out to take accident video 4) notice a lifted load with no crane operator around a) coach crane operator not to leave lifted load unattended b) walk under the load, its in your path 5) while lifting plate, it slips and you notice worn teeth on the plate dog a) realizing you should have inspected it, you now put it out of service b) you leave it in service and let someone else use defected plate dog
