Use ophthalmoscope and assess pupillary response, OCULOMOTOR (III), TROCHLEAR (IV), ABDUCENS (VI) NERVES - Using an H pattern have the patient follow with their eyes only. , GLOSSOPHARYNGEAL (IV), AND VAGUS (X) NERVE - Look inside patients mouth, have them say 'ahhh', then state gag reflex was assessed, HYPOGLOSSAL NERVE (VII) - Have patient stick tongue out and move it left and right, SPINAL ACCESSORY NERVE (VI) - Have patient shrug shoulders against resistance.,
