504 Chair/ Co-Chair, Provide annual 504 overview for faculty and staff, Chair all 504 meetings (initials, annuals, manifestations, reviews), Adhere to Federal Law and City Schools’ 504 processes and procedures, Serve as the school’s initial point of contact for inquiries and referrals, Manage and organize all 504 documentation and folders (referrals, eligibility, assessments, progress reports, meeting notes, 504 plans, parent communication, etc.), Attend City Schools’ mandatory quarterly professional developments/training, Work with school administration to ensure 504 plans are implemented, Administrators, Identify a staff member(s) to serve as the 504 Chair for the current school year. Principals can identify additional staff to serve as 504 Co-Chairs as needed to support the caseload management., Address parent concerns with the Section 504 process., Ensure articulation of activities to include the appropriate transfer of the student’s Section 504 information and/or folders are adhered to, Ensure the appropriate inclusion of the student with disabilities in field trips and/or extracurricular activities., Ensure that Section 504 plan documents are uploaded into Infinite Campus and plans are distributed to all employees who will provide Section 504 services to students., Ensure the appropriate discipline procedures for students with disabilities are followed., Ensure 504 plans are being implemented as written, Teachers, Sign receipt of all 504 plans received, Implement 504 plan as written for students with a plan, Document implementation (lesson plan, accommodation matrix, etc.) of accommodations/supports you provide to student from the 504 plan, Complete 504 progress reports as required for 504 meetings, Active 504 team member (when appropriate).
