Cervical Compression Test (Spurling Test), Seated. Look over one shoulder, then look up at the ceiling. While standing behind the patient, apply downward pressure on their head. Positive = neck pain with radiation on the same side of the turned head., Cervical Distraction Test, Supine. Place one hand on the patient’s occiput and the other hand either on the occiput/ forehead/under the chin. Pull the patient’s neck away from the patient’s body. Positive = cervical if the pain is RELIEVED., Cervical radiculopathy tests, Cervical Compression Test (Spurling Test) & Cervical Distraction Test, Lhermitte Sign/Lhermitte Phenomenon/Barber Chair Phenomenon, Seated. Patient to flex their neck down. Positive: If the patient feels a transient shock-like sensation radiating down the spine and/or upper/lower extremities. May occur in several conditions, such as multiple sclerosis, spinal cord compression/tumor/injury, or radiculopathy, Adson Test, Palpate the radial pulse on the patient’s elbow fully extended. Have the patient rotate their head towards the same side, then extend their neck. While still palpating the pulse, abduct and laterally rotate the patient’s shoulder. Ask patient to take a deep breath and hold it. Positive: decreased vigor of the pulse., Straight Leg Raise Test, Supine. Place one hand under the patient's ankle, and one hand on the anterior-superior knee or hip. Flex patient's extended leg to the trunk at the hip. Positive: patients’ sciatica symptoms are recreated at 30-60 degrees. (Pain shooting down the leg). Dorsiflex the foot - worse., Sitting Knee Extension Test, Sitting. Flex their neck (chin to chest). Fix the thigh on the bed with one hand and extend the leg with the other hand. Positive = pain, worse with dorsiflexion., Sciatica tests, Straight Leg Raise Test & Sitting Knee Extension Test, Test for thoracic outlet syndrome, Adson Test
