It is used to record all the operational details – including crew, date, destination, no of pax., cleaning etc., Cabin Safety Report - Used to keep a record of any significant events/problems during the flight – could relate to catering, seating, cleaning, pax. complaints, medical incidents, behavior problems, delays and any situation that can affect cabin crew., Cabin Defects Log/Record - Used to report any defect/faults found by cabin crew on the aircraft - e.g. seating, lighting, galley equipment, toilets, safety equipment, in-flight entertainment. A qualified engineer signs all repairs made., General Declarations - A list of all crew, a note of any sickness and disinfestations is prepared for each flight, signed by the Captain and handed to the authorities upon arrival., Crew Performance Report - Completed by SCCM for every crew member, giving details of how well they have completed their duties - important for improving standards and assessing staff for promotion prospects., Crew Declaration - This is a customs document completed by crew travelling to the UK from a non-EC destination. Each crew member signs to acknowledge they are not bringing goods into the UK that exceeds their allowance. This is a legal document and crew can be prosecuted if the form is completed incorrectly., Disruptive Passenger Form - Used in the event of a pax. breaching the Air Navigation Orders. Crew complete this form to record a full account of what happened., Air Safety Report (ASR) - Completed if a Pax. or crew member has a serious accident/illness on board the aircraft., Cleaning Specification Form - Some low-cost airlines require cabin crew to carry out a quick clean on turnaround. However, most charter airlines still require a full ground service cleaning specification., Bar Paperwork (C209) - Completed by cabin crew for HM Customs and bonded store. It is a legal document that must be signed by the person completing it. This from documents all the flight details, aircraft registration, bar number and a list of all the goods on board, Summary of Sales - Used by the airline accounts dept. to ensure the correct monies have been taken. The Cabin Crew details are also on this form to ensure commission is paid.,
